HEALTH Program

Salvation Army Shelter (630 East Washington Avenue, Madison, WI)

The HEALTH Program meets at the Salvation Army shelter the 2nd Monday evening of every month from 6:00-8:00pm

Drop-In Shelter at Grace Episcopal Church (116 West Washington Avenue, Madison, WI)

The HEALTH Program meets at the men's drop-in shelter the 3^rd Wednesday evening of every month from 5:30-7:30pm from November through February. 

From April through October, patients will be transported from Grace to the Hut at 5pm.

Luke House Meal Program (310 South Ingersoll, Madison, WI)

The HEALTH Program meets at Luke House the 1^st and 3^rd Wednesday every month from 11:30am-1:30pm. 

Holy Redeemer Meal Program (190 West Johnson, Madison, WI)

The HEALTH Program meets at Holy Redeemer the 2^nd Friday every month from 12:30am-2:30pm. 

The HEALTH Program will be available Monday-Friday to follow up individual patient care coordination needs of Safe Haven, Hospitality House, YWCA and Brooks Street clients.  Contact us to arrange follow up for specific patients.
